SHC Medical Partners, LLC is seeking an experienced healthcare professional to become a part of our team as a Care Navigator! You will focus on providing high quality customer service as part of a healthcare team.

Summary: Under the supervision of the COO, the Care Navigator (CN) serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services within SHC Medical Partners Hub of Excellence and in the community. The CN assists in the overall care coordination, data entry, metric tracking and communication pathways to help facilitate a person-centered, safe, efficient, and effective care transitions for all Hub of Excellence patients across the continuum of care. The CN will serve as the lead liaison for the Hub of Excellence team. The position’s responsibilities will involve working from existing project tools and processes to developing new tools based on knowledge and experience along with being a collaborative liaison for all levels within the company (executive team, clinical operations, clinical team, recruiting, etc.) and its clients.

Job Requirements:

· High School Diploma required.

· Advanced education in healthcare preferred.

· Minimum of two (2) years’ experience in long-term care preferred.

· Understanding of the health care system and its components including sites of care, delivery models, and the roles of various providers and health care professionals.

· Experience in EHR along with workflow and practice management.

· Experience with healthcare operations and clinical workflows.

· Effective verbal and written English communication skills.

· Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation.
