How you will make a difference
* Responsible for the direct leadership, oversight, and accountability for the operational effectiveness of Case Management.
* Executes the strategic plan of Case Management, partnering with Case Management leadership to ensure alignment with the organization’s metrics and goals.

* Identify and participate in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment.
* Management duties including, but not limited to, hiring, training and developing, coaching and counseling, and terminating department staff, as deemed necessary.
* Ensure that all employment practices are administered fairly and without regard to race, color, religion, gender, sexual orientation, national origin, age, disability, marital status, amnesty, or veteran status in accordance with applicable federal, state and local laws.
* Establishes organization’s operation standards and policies for case management review.
* Develop, implement, coordinate, and evaluate health care plans in an in-patient and out-patient environment with other health care providers and community service agencies to coordinate care to our specified client group and their families, while also monitoring contractual reimbursement to net maximum profitability.
* Coordinate, monitor, and evaluate provider services and care, to include home and/or physician/hospital visits, and therapy visits, for a specified client population.
* Facilitate client/family access to health care and community resources; assist client/family to develop natural resources and make contact with social support networks; develop resources as appropriate to meet client/family needs in their community.
* Perform a range of associated administrative functions, as appropriate, such as monitoring budgets, preparing administrative and programmatic reports and correspondence and submitting billing documentation, as required.
* Travel to outreach locations to meet with clients/families, coordinate services, and attend regular staff and management meetings.
* Assist with clients and/or families to assess, develop, implement, monitor, and recommend modifications to comprehensive, cost-effective care plans, using a multidisciplinary process.
* Apply management and clinical expertise, education, research, and consultation to optimize clinical/operational outcomes.
* Develop supportive working relationships with other segments, Home Office executives and staff to ensure high quality patient care.
* Other special projects and duties, as assigned.
The qualifications you will need
* Must be a Certified Case Manager (CCM).
* Current licensure as a Registered Nurse in state employed.
* Minimum of three (3) years’ experience with alternative payment methods experience.
* Ability to develop, implement, evaluate and modify therapeutic programs and services.
* Knowledge of billing procedures for clinical and social services.
* Knowledge of clinical and social services documentation procedures and standards.
* Experience in data analysis, program evaluation and strategic planning
* Knowledge of community health services and social services support agencies and networks.
* Knowledge of state and federal regulatory requirements.
* Effective verbal and written English communication skills.
* Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation.
* Highest level of professionalism with the ability to maintain confidentiality.
* Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
* Customer service oriented with the ability to work well under pressure.
* Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
* Strong analytical and problem solving skills.
* Ability to work with minimal supervision, take initiative and make independent decisions.
